Treatment of Diabetic Foot Ulcers

Management of Ischemia

Patients with diabetic foot ulcers that may be attributed to peripheral vascular disease (ischemia) should be promptly referred to a vascular surgeon for further evaluation and testing. Evaluation of suspected peripheral arterial disease usually includes:

  • Ultrasound

    • measurement of ankle pressures
    • measurement of toe pressures
    • duplex scanning
  • Imaging studies with contrast angiography

    • digital subtraction angiography
    • magnetic resonance angiography
    • CT angiography

Treatment options for the management of peripheral arterial disease include:

  • Vascular reconstruction to restore sufficient blood flow to the affected limb
  • Angioplasty and stenting
  • Hyperbaric oxygen treatments
